Your life, accompanied.
Accompany is a small, personal geriatric care management practice in New York City. We help older adults live safely and well, and help their families figure out what to do next, with a registered nurse personally guiding every step.
What we do
Care management, handled personally
Most families reach out when something has changed: a new diagnosis, a hospital stay, or a parent who needs more help than the family can provide alone. My work is to figure out what needs to happen, put a plan in place, and follow through, at that moment and as things change afterward.
Care assessment & care planning
Every engagement begins with a thorough, in-person assessment (health, cognition, home safety, current support, and what matters most to your family), distilled into a written care plan you can act on and share.
Care coordination & provider management
One point of contact across physicians, home care, therapists, and family. We schedule, attend, ask the follow-up questions, and make sure nothing agreed in an exam room gets lost on the way home.
Dementia care guidance
Practical, unhurried guidance as memory and thinking change: safety at home, communication that still works, support for caregivers, and planning ahead while your loved one can still take part in the decisions.
Hospital-to-home & post-surgical transitions
Discharge is where good plans most often come apart. We prepare the home, reconcile medications and instructions, arrange follow-up care, and stay close through the first days back.
End-of-life & hospice support
When cure is no longer the goal, comfort and dignity are. We help families understand their options, coordinate with hospice, and make sure a loved one's wishes are heard and honored.
Ongoing monitoring & advocacy
Regular visits and watchful follow-up from a nurse who knows your family well, noticing small changes early, and speaking up in the rooms where decisions get made.
About the practice
I want to stay in my own home for as long as I can. I want the same for my family, and for everyone I love. Most of my clients want exactly that: their own home, their own routines, their own life, for as long as possible. I started this practice to help them have it. What usually gets in the way is not love or willingness. It is the pile of practical things: aides to hire, medications to keep track of, appointments to make, and problems that show up without warning. That is the part I take on.
The name Accompany comes from my first training, as a classical musician at Yale. The best accompanists I worked with were never the story. They made the person in front of them able to do their part. I try to do the same thing for families.
As a registered nurse, my clinical work has centered on the situations families find hardest: dementia, the end of life, recovery after complex surgery, and the fragile handoffs between hospital, rehab, and home.
I keep the practice deliberately small. When you work with Accompany, you work directly with a nurse who knows your family, your physicians, and your plan.
- Dementia care: guidance from first concerns through advanced disease
- End-of-life & hospice support: comfort, clarity, and advocacy
- Complex post-surgical recovery: structured, closely monitored healing at home
- High-acuity care transitions: hospital, rehabilitation, and home, without the gaps

Getting started
It starts with a goal
For most clients, the goal is to stay home safely. For others it is getting through a recovery, keeping a husband or wife at home, or being comfortable and present at the end of life. The care plan is built around that goal, and every decision after it is measured against it.
A conversation
We begin with a phone call: what is happening, what you are worried about, and what your goals are.
Initial assessment
A comprehensive in-person assessment, followed by a written care plan with concrete recommendations.
Ongoing care management
From there, we put the plan into motion and stay with it.
